Immunization Forms and Records

MMR (Measles, Mumps, Rubella) immunization records and the Meningococcal Meningitis Vaccination Response Form are required prior to registration.

1. Please complete the Hunter College’s Immunization Record Form (see link below).
   You will not be able to e-sign the PDF if you open it in your internet browser.
   To fill and sign the PDF digitally, please download it to your computer first, and open it using Adobe Acrobat.
2. Upload it on CUNYfirst via the Document Uploader.
   Only online submissions are accepted. Do not submit paper forms.
   1. Include your name on each page of the documents.
   2. Scan your records into PDF format before uploading them. We do not accept photos of immunization records from mobile devices.
   3. Review your scans. Forms and photo IDs that are not legible cannot be processed, and this will delay your registration.
   4. Submit your documents via the CUNYfirst Document Uploader:
      • Access your CUNYfirst account
      • Select the HS Immunization Record category.
   5. After you have uploaded your records, email us at wellness@hunter.cuny.edu to confirm that we received them. Include your full name and CUNYfirst ID/Student ID in your email.

Any student whose religious belief prohibits vaccination must complete the CUNY Religious Exemption Form (see link below). Exempted students will not be permitted on campus during a communicable disease outbreak.

Do you need a copy of your immunization records or would you like them to be sent to another institution? Immunization Records can release your immunization records in one of the following ways:

• You can pick up your records in person at Immunization Records, Room 307, North Building (a valid photo ID is required)
• Or you can email wellness@hunter.cuny.edu, or mail the Consent to Release Immunization Form. If you are e-mailing the form, it must be scanned in a PDF format and you must attach a valid and legible photocopy of your ID. No mobile snapshots of IDs will be accepted.
• Download the Consent to Release Immunization Records Form below.
