Proof of Immunization Due

Students are assigned WA (Administrative Withdrawal) grades for non-compliance with MMR immunization requirements. You must provide proof of receiving the Measles, Mumps, and Rubella vaccines, and sign the Meningitis Acknowledgement Form.
