Health Requirements and Clinical Clearance (Undergraduate)

Prior to beginning any clinical placements, students must submit required health and professional clinical practice clearance documents by the deadlines announced by the School. A satisfactory background check and drug screen are also required.

Health Clearance

The HBSON (and CUNY) use CastleBranch to monitor and maintain student health clearance requirements for clinical practice. All students are required to have a current CastleBranch account.

Health clearance packets are available below. Instructions for how to create a CastleBranch account are provided on page 2 of the packet. Please carefully review all information regarding where and when to submit required documentation. Students will not be permitted to begin any course with a clinical component if these requirements are not met by the required date.

Health Requirements and Clinical Practice Clearance Forms
Version #22-05. Always check that the version number matches the version shown on the bottom-left corner of your forms’ cover page. Failure to use the most current version of these clearance forms may result in your submission being incomplete and regarded as late.

Please retain a copy of each document and keep a set available to you while in the clinical setting. Students are responsible for submitting updated clearance documents with any health status changes that may impact patient safety, including, but not limited to, mental health changes or taking medications that may impact alertness. Faculty advisors or administrators may also request updated clearance documents from students in whom they assess a health status change that may impact patient safety.
