Immunization Requirements

You must submit proof of MMR (Measles, Mumps and Rubella) vaccinations and the Meningococcal Vaccination Response Form if you are an incoming student taking 6 or more credits. Failure to comply will result in an Administrative Withdrawal grade from all classes.

Current Students — You have until Friday, Sept 27, 2024 to submit proof of immunization.

New Students — You must submit proof of MMR (Measles, Mumps, and Rubella) Vaccinations and the Meningococcal Meningitis Vaccination Response Form prior to registering for courses, if you are an incoming student taking 6 or more credits.

International Students — You have until Saturday, October 12, 2024 to submit proof of immunization.

Before you can register for your classes, you must FIRST submit proof of your immunization records, as required by New York State Public Health Law 2165.

You are required to provide legible proof of:

• MEASLES (Rubeola) – Two doses of Live Measles Virus Vaccine at least 28 days apart: on or after 1st birthday and dated 1968 or later. (For more information visit the NYC Health website)
• MUMPS – One dose of Live Mumps Virus Vaccine: on or after 1st birthday and dated 1968 or later.
• RUBELLA (German Measles) – One dose of Live Rubella Virus Vaccine: on or after 1st birthday and dated 1969 or later.

OR

• MMR Vaccination (Measles, Mumps, Rubella – 2 doses after 01/01/1972
  Dose #1 must be given at age 12 months or later
  Dose #2 must be given after 15 months of age
  – OR at least 28 days after first vaccine
  – OR Any MMR diagnosis must be accompanied by a titer test (especially rubella).

Documentation

Proof of these vaccines must be documented in one of the following ways, at least three to four weeks prior to your registration appointment:

• Have your health care provider fill out, sign, stamp and attach your immunization documentation to our Immunization Form,
• Provide a signed and stamped copy of your health care provider’s or clinic record,
• Provide an original signed and stamped copy of your lifetime immunization record card (childhood yellow/blue immunization card), or
• Provide an official signed and stamped copy of immunization records from a former high school, college, employer or other institutions you attended.
• Immunization records can be obtained from a public health department immunization information system. Students born after 1994, who were raised in New York City can check the Citywide Immunization Registry for their records by calling 311.
• Provide a dated positive blood test lab result (known as a titer or serology) with reference ranges showing immunity to measles, mumps and rubella.
• Veterans — Proof of honorable discharge from the armed services (Form DD214) within 10 years from the date of application will enable the student to attend school pending actual receipt of the immunization records from the armed services.

New York State Public Health Law 2167 requires that all college and university students, regardless of age, who are enrolled for at least six (6) semester hours or the equivalent per semester, must complete and return the Meningococcal Vaccination Response Form prior to your first semester. This information appears on Part 3 of the Immunization Form. Failure to submit this form will delay your registration.

How to submit your Meningococcal information:

1. Access your CUNYfirst account
2. Navigate to Self Service, then to Student Center
3. Under the To Do List tab, click on Meningitis Acknowledgement Form
4. You will be able to review information regarding the Meningococcal Meningitis vaccine, and respond electronically.

The Meningococcal Meningitis vaccine is not mandatory, but we require proof that you have received the vaccine within the last five years before the meningitis hold can be removed. Students who choose to decline the vaccine online or on the Hunter College Immunization Record Form, by selecting the third option on the form, will have an immediate removal of the hold.

If you are unable to respond electronically, you can answer by completing Part 1 and Part 3 of the Hunter College Immunization Record Form (below) by scanning and uploading it via the Document Uploader in CUNYfirst. Proof of meningitis vaccine must be attached if you choose not to decline.

If you received your vaccinations outside the United States, your foreign vaccine-related immunization records must be translated into English prior to submission to Immunization Records. Failure to do so may delay your registration.

You must submit foreign immunization information at least three to four weeks prior to your registration appointment. Failure to do so may result in registration restrictions. We make every effort to process the forms as quickly as possible. But we cannot guarantee your registration if your records are not submitted in a timely fashion.

Medical — A letter from your licensed physician/health care provider is required indicating the medical reason and length of time for contraindication. This letter must be written on the provider’s stationary with stamp, license number, and signature.

Religious — Submit a written statement of sincere and genuine religious objection to immunizations. The determination regarding exemption is made following review of your written statement. Learn more on the Forms and Records page.

Birthdate — All students born prior to January 1, 1957 are exempt. However, students enrolled for at least six (6) semester hours or the equivalent per semester complete and return the Meningococcal Vaccination Response Form.

Online only — Students registered for online courses only and will not meet on campus or any school fieldwork/practicum are not required to meet the immunization requirements.

If you have changed your name, and wish to change you name in College records, your request must be accompanied by supporting documentation (for example: Marriage License, Court Order, Divorce Decree, Birth Certificate, Immigration Papers). Immunization clinics are available outside of Hunter College. Students currently enrolled in a post-secondary educational program are eligible to receive a free MMR vaccine through the Department of Health in order to fulfill school requirements.
